I choose the Countryconnect as my primary long-distance provider for the service(s) and telephone number(s) indicated below and I authorize the Countryconnect to act as my agent by notifying my local telephone company of this choice. I certify that I am legally responsible for the payment of charges incurred on the telephone number(s) listed below and that I have the authority to change the prescribed 1+ long distance carrier providing service to these numbers. Further, I recognize that I can have only one primary long-distance company for a given telephone number, that I will no longer be presubscribed to my current long-distance carrier's service(s) and that my local telephone company may impose a charge for this and any later change. I also understand that the Countryconnect's name will appear in my long-distance bill as a result of this switch and that this change only affects my 1+ long-distance services. I understand that my signature on this form means the following services will be provided to me by the Countryconnect: IntraLATA(when available); InterLATA, interstate and intrastate telecommunications.
In order to protect you (and us) from slamming, the Federal Communications Commission (FCC) has recently made it a requirement for consumers to fill out a document authorizing a switch in your long-distance service. Countryconnect must have a completed "Letter of Agency" before it can switch your long-distance carrier.
"Slamming" is when a long-distance carrier switches your service without your knowledge. This practice may involve deceptive marketing tactics or outright fraud, and it could affect your local service. When you are "slammed," you could lose important service features, get lower quality service or be charged higher rates for long distance without you knowing. Because of the increase in "slamming" over the years, the FCC requires long-distance companies to obtain authorization before changing your long-distance service provider.

IntraLATA - Calls to or from locations within your LATA. Also sometimes called "local long distance" calls. InterLATA - Calls to or from locations outside of your LATA, which usually includes almost all interstate calls. Both - Refers to signing up for both IntraLATA and InterLATA service for the phone number listed.
